A 2 year-old boy was presented to us with: paleness, stomach aches, obstipation, urination difficulties and a poor general condition. In the lower abdomen, a large mass could be palpated. The child was transferred by his pediatrician to pediatric surgery with the suspicion of obstipation.  

ALL with blasts in the peripheral blood and bone marrow. In the ultrasound, a large space-occupying lesion in the pelvis with blockage of the kidneys and surrounding of the rectum was noticed. A rare manifestation of a large lymphoma in the pelvis.

Initially, the child was given an artificial anus, both kidneys were drained to the outside and a suprapubic bladder catheter was placed because of increasing retention of uric substances.
