 | Age: | 15 Years | |
 | Gender: | Male | |
 | Region-Organ: | Leg-Bones | |
 | Most likely etiology: | physical | |
 | History: | 15 year-old boy, findings after a skateboard accident, torsion-bending trauma of the right distal lower leg, strong swelling, pain and hematoma. | |
 | Pathomorphology or Pathophysiology of this disease : | N/A | |
 | Radiological findings: |
<- view X-Ray 1
X-Ray 1: Distal lower leg a.p. : splintered spiral fracture of the distal third of the fibula. Fracture gap in the epiphysial mid-section of the tibia without development of a step.
<- view X-Ray 2
X-Ray 2: Distal lower leg lateral: Depiction of a small meta-epiphysial fragment dorsolaterally in the sense of a transitory fracture.
<- view X-Ray 3
X-Ray 3: X-Ray fluoroscopy. Better depiction of the fragment in diagonal rays. Step development.
<- view CT 1
CT 1: Larger epiphysial fragment laterally, gaping fracture gap, incomplete fracture of the epiphysis in a 90° angle to the main fracture.
<- view CT 2
CT 2: CT futher proximally, Depiction of 2 metaphysial fragments in the sense of a triplane I fracture.
<- view CT 3
CT 3: CT-Findings postoperatively after screw fixation.

 | Diagnosis confirmation: | Imaging including endoscopy | |
 | Which DD would be also possible with the radiological findings: | N/A | |
 | Course / Prognosis / Frequency / Other : | After inital diagnostics with conventional x-ray and further vagueness about the course of fracture, depiction through CT. Surgical treatment with screw fixation. Intrapoerativally, the fracture gap in the tibial articulation was difficult to assess, therefore postoperative follow-up CT. | |
 | Comments of the author about the case: | Because of the second line of fracture, the situation of an "incomplete double triplane I fracture" arises. | |
